Please follow the example below. Please proof your list, if there is an extra space or mistake please fix, because how you type it is exactly how it will be printed. Not all characters, such as an ampersand (&), can be in different fonts. It's best to type everything out in full. And if a zip code begins in 0 please make sure your list is not set to rounding up numbers, otherwise the 0 will drop off. 

Please include country in a column beside zip only if you have a card mailing out of country.  If mailing in the US, please leave blank.
